Intraoperative Ultrasonographic Imaging of the Left and Middle Hepatic Veins



Abstract
An accurate knowledge of the surgical anatomy of the hepatic veins and the infrior vena cava is indispensable for extensive hepatectomy, transplantation of the liver and the treatment of hepatic trauma with avulsion of the hepatic veins. Attempts
at the
classification of the patterns of ramifications of the middle hepatic vein(MHV) and the left hepatic vein(LHV) have been accompanied by greater descriptive than those of the right hepatic vein (RHV) due to the anatomical variations.
The surgical anatomy of LHV and MHV was examined by intraoperative urasonography (IUS) in thirty-two patients who underwent an exploratory lapalotomy from August 1991 through April 1993. The results of the present study demonstrate that IUS can

classify the various branches of the LHV and MHV; 2) identify those venous branches that can safely be ligated grior to transection of the liver parenchyma: and 3) assess the utility of such information in partial liver transplantation.
